Now that the summer break is over and the kids are back in school, it’s time to get back into a routine that focuses on your health. Over the last few months it was easy to stay up late, and enjoy more than usual amounts of pizza and ice cream. Who hasn’t been there?

 

Here are some tips to help you get back into a healthful pattern that keeps you at your peak performance. We encourage everyone to be your best so you can take care of your family, not the other way around.

1. Rise early and get your workout in

Working out first thing in the morning does a lot of great things for your health. Elevating your heart rate and moving around helps remove toxins, drains your lymph nodes and aids in regular digestion, to name a few. Energy levels are higher and fat burning is accelerated. Focus and motivation are stronger and stress is lowered. The list goes on and an on of the wonderful benefits.

2. Get to bed by 10 pm

In order to get up early for your workouts (and getting the kids off to school) you need to get to bed early so you can wake up early. Study after study has shown that early risers are more productive, healthier and happier individuals.

3. Master portion control

If you are already waking up early and exercising, then this one is much easier. Portion control is simply the practice of eating less than you think you need. A simple way to measure: Make a fist, then consume no more than 80% of that volume in food. Your fist is approximately the same size as your stomach and an easy way to determine food portions. For stable blood sugars, we recommend following a Low Glycemic Nutrition Plan.

4. Hydrate for power

With all the different choices out there for drinks, water should always be your #1 go to. Anything that contains sugar, alcohol or caffeine works to dehydrate, not hydrate you. Staying hydrated is essential to help your body flush toxins, keep your skin young and firm, keep a clear head, more energy and so much more.

5. Love your workout

When it comes to working out, do what you can handle. Lift weights if your physical health allows it, or run on the treadmill. Talk a walk. Go for a swim. Some exercise is better than no exercise! If you are having trouble finishing or recovering from your workouts, it may be a sign of low testosterone.

6. Be accountable

One of the easiest ways to show up is having someone else depending on you to be there. Don’t be afraid to be accountable.

7. Buddy up

Having a friend work along with you helps you both succeed in meeting your goals. Having a buddy helps provide motivation, accountability and a feedback system to know you’re on the right path.

8. Be consistent

Regular execution of the positive behaviors mentioned above will help form them into habits. Before you know it, you will feel out of place eating poorly or not exercising. Consistency is the key!

How To Have A Guilt Free Oktoberfest

With all the great food and drink at Oktoberfest, how do you enjoy the festival without wrecking your health and fitness goals?

These practices are simple and easy to follow, and apply to enjoying yourself at any festival. Especially if you’re over 30, we don’t recover like we used to from a night of partying.

  • Go grilled – this is a clean eating option vs. the fried foods, sweets and heavy sauces.
  • Stay hydrated – for every alcohol drink, have a bottle of water. Try to limit your beer or wine to 2-3 total.
  • Move around – you will feel better, plus you can check out all the sites, vendors and attractions!
  • Limit the deserts – have small amounts or split it with friends. There is no need to eat 1000 calories in one sitting 😉

How about the day after?

Build A Better Breakfast

What if we could directly control our metabolism through the foods we choose to eat?

Getting through the day with plenty of energy can be done in one of several ways. While energy drinks and quick sugary snacks help for a short while, the constant ups and downs in your blood sugar create serious damage to your body. Before you know it, you’re taking naps, experiencing low mood and need more quick calories to get you moving again. Even in the short term, you fuel the process of glycation which leads to increased fat storage and decreased metabolism. Long term, this eventually leads to metabolic syndrome – a life threatening condition affecting millions of Americans.

Some individuals like to skip breakfast, which lends itself to the desire for quick energy foods as mentioned above. Unless you are working on a controlled fast, foregoing the first meal of the day is not in your best interest if you want smooth, consistent energy throughout the day. Let’s get into the proper way to start your day.

Are all calories created equal?

This is where many people are confused and I at one time was no different! It turns out that this is not the case. Thanks to the field of nutrigenomics we now understand that all calories are not created equal and their effects on the human body can be quite dramatic and even have different effects on different people.

Studies have now revealed that diets made up of high glycemic carbohydrates (rapidly absorbed sugars) spike your blood sugar levels which in turn spike your insulin levels. Constant high insulin levels promote fat gain, high triglycerides and cholesterol, insulin resistance, fatty liver, obesity, inflammation and all the disease associated with it.

So let’s play a game I call “Pick the Better Breakfast”. We determine which one of these three breakfasts will supercharge your body giving it all the vitamins, minerals, and nutrients that it needs to power up your day all while keeping your blood sugar levels stable. Here is the tricky part; they are all “healthful foods”.

A. Scrambled eggs with diced peppers on a bed of arugula with 10-grain bread sliced avocado and one cup of berries.

B. Steel cut oats with added flax, walnuts, and a cup of berries.

C. Rolled Oats with a cup of berries and a glass of orange juice.

Picking the right one can make a huge difference as to whether you gain weight or lose weight. Seems pretty straight forward but there is actually an enormous difference in the way your body metabolizes the different foods you choose to eat each and everyday.

Foods that are digested quickly enter the bloodstream quickly, which means higher blood sugars, which can mean a slow metabolism and weight gain. Compared this to a food that is digested slowly, which means more stable blood sugars, which means weight maintenance or loss. In addition some of these foods have anti-inflammatory properties and others inflammatory properties.

Which is better for you: apple juice or an apple? Seems straight forward enough, since they are both made of the same food material, right? Well, one enters the bloodstream very quickly because of its processing, as the apple juice has had one of the important parts, the fiber, removed from it. However the apple in its natural state contains all of the vitamins, minerals, phytonutrients, antioxidants, and fiber! Everything your body needs. The rate at which the apple is digested is slower, which means your body has time to utilize the energy from the apple.

The lesson on the state of the apples:

  • Eating unprocessed apples promotes weight loss.
  • Drinking apple juice can promote weight gain or hinder your weight loss efforts.

In our examples of the 3 different breakfasts, they are all healthful foods; that is where the similarity ends. You see each one of these breakfasts will be digested at different rates resulting in either rapid or slow escalation of blood sugar.

That difference alone can make all the difference in your overall health and in the aging process. By maintaining stable blood sugars through out the day you promote health, wellness, consistent energy, and utilization of digested and stored energy.

So which breakfast is our winner? While breakfast C is certainly healthy, its consumption results in consuming roughly 80% more food later in the day than breakfast A. Breakfast B is certainly a healthy way to start the day, but its consumption led to eating 51% more food later in the day than breakfast A. Which makes breakfast A our winner!

The combination of eating eggs, peppers, arugula, avocado, and berries was not only a breakfast with a low sugar content, but one with anti-inflammatory properties as well which means stable blood sugars, fats, insulin, and adrenaline levels. This group of foods sends a hormonal message to speed up your metabolism.

The interesting fact here is that the types of food we eat not only supply our bodies with energy they also send signals to either slow down or speed up our metabolism. Which is great news because we can directly control our metabolism through the foods we choose to eat.

Seven Tips to Naturally Increase Your Testosterone

Testosterone is the major sex hormone in the human body. It’s production starts in the brain and ends in the testes in men and the ovaries in women.  The adrenal glands located on top of the kidneys also produce testosterone in limited amounts.  The brain and the testes work in concert to keep production in the normal range.  When testosterone levels are low the brain signals the testes to make more and when there is enough testosterone, the byproducts of testosterone production signal the brain to stop.  The regulation of this production referred to as the Hypothalamic-Pituitary-Gonadal Axis can be disrupted at any point causing either primary or secondary hypogonadism.  Provided you have and intact H-P-G axis and sufficient leydig cell function in the testes to produce testosterone in the first place these seven tips can help boost your body’s natural production of testosterone.

1.  Eat Healthy Fats

The consumption of health fats encourages and is necessary for the production of adequate testosterone while a low fat diet will reduce testosterone levels. Consume these healthy fats on a regular basis to ensure hormone production:

  • Walnuts
  • Almonds
  • Pistachios
  • Olive oil
  • Flaxseed
  • Avocado
  • Salmon, Tuna, Trout, Sardines

 

2.  Follow a Low Glycemic Nutrition Plan

The cornerstone of all health is nutrition.  Consuming foods that cause high blood sugar spikes will drop your testosterone production and the resultant high insulin levels from the high blood sugar will compete with growth hormone for the same receptor site.  Other benefits of following low glycemic nutrition plans are:  consistently high energy levels, eating satisfaction, efficient immunity, maintenance of healthy weight, and potential for increased longevity.

When a low glycemic meal is consumed, blood glucose elevates at a slower pace, allowing the pancreas to moderate the amount of insulin released so it is relative to the volume of glucose. Because there is less insulin circulating in the blood stream, fewer cells absorb glucose; levels never fall below optimal range. Food cravings and low energy crashes diminish once you adjust to the low glycemic diet, as the body is consistently able to utilize body fat as its primary source of energy.

Consuming balanced low glycemic meals maintains a consistent balance of hormone levels, allowing the body to function efficaciously with optimal health.

A low glycemic nutrition plan is not only important for the reasons indicated above, but also because:

 

Insulin Competes with Growth Hormone

These two hormones use the same receptor site to communicate with cells; therefore, only one hormone can be functionally elevated at a time. Managing insulin with a low glycemic diet will allow growth hormone to be optimally high and functional.

Healthy frequent meals equal steady state glucose and insulin levels, increased metabolism and decreased cravings. Steady state glucose and insulin levels equal increased fat utilization and decreased fat production.

The other side of the coin!

When carbohydrates are consumed, the body breaks down, absorbs, and transports them via the bloodstream in the form of blood glucose to the cells for use as energy. However, most cells cannot absorb glucose without the presence of insulin, a hormone that stimulates cells to absorb the glucose. Therefore, when blood glucose rises, so does insulin. This is a good, natural, physiological process that only becomes problematic when high glycemic foods are consumed on a regular basis:

High glycemic foods increase blood glucose rapidly, resulting in high insulin levels. Insulin is a “storage” hormone. It not only promotes the absorption of sugar, but, because there is already enough free energy in the bloodstream in the form of glucose, insulin inhibits cells from releasing their stores of fatty acids. Fatty acids comprise stored fat tissue. Therefore, when insulin levels are high, the body cannot utilize body fat as an energy source.

Insulin does not immediately dissipate as glucose levels fall. In the presence of insulin, cells will continually absorb glucose, causing blood levels to fall to an unhealthy level. Because the brain can only utilize glucose for energy, food cravings and low energy become typical experiences.

 

Effects of a high glycemic diet:

  • Fluctuating energy levels
  • Excessive food cravings
  • Lack of satiety from eating
  • Easy onset of fatigue
  • Weight gain
  • Increased abdominal fat which leads to an increased risk of Metabolic Syndrome
  • Increase in cellulite
  • Decreased testosterone production

 

3. Supplement with Vitamin D

When Vitamin D levels decline during the fall and winter months, so do your testosterone levels.  Supplementing with Vitamin D during these months will help ensure adequate hormone production.  If you live north of Florida than you most likely are vitamin D deficient.  It is the most common vitamin deficiency I see on a daily basis and the effects from supplementation are profound.

 

4.  Consume foods with Zinc

Low levels of zinc are associated with low levels of testosterone.  Be sure to consume these foods to help boost your T levels!

  • Nuts, especially cashews
  • Dark chocolate
  • Spinach
  • Seafood like crab, lobsters, and shrimp
  • Lamb

 

5.  Run Sprints or High Intensity Interval Training (H.I.I.T.)

Running sprints or short bursts of intensity lasting approximately 6 seconds dramatically increase the levels of testosterone and the catecholamines: adrenaline and noradrenaline.

A HIIT session consists of a warm up period of exercise, followed by six to ten repetitions of high intensity exercise, separated by medium intensity exercise, and ending with a period of cool down exercise. The high intensity exercise should be done at near maximum intensity, 90%. The medium exercise should be about 50% intensity. The number of repetitions and length of each depends on the exercise. The goal is to do at least six cycles, and to have the entire HIIT session last between fifteen and twenty minutes. HIIT is considered to be an excellent way to maximize a fat loss workout. HIIT increases the RMR, resting metabolic rate, for the following 24 hours. HIIT turns on your metabolic fire! Specific hormones that are released during this type of training are growth hormone and catecholamines, which contribute significantly to decreases in body fat. Catecholamines are fat destroying hormones that have been shown to liberate both intramuscular and subcutaneous fat. Steady state exercise only results in small increases in catecholamines. HIIT is a powerful stimulator of growth hormone. Growth hormone limits lipoprotein lipase, which is a fat storage enzyme, excellent. What is another powerful stimulator of GH, getting a full night of sleep. HIIT has been extensively studied and is significantly more effective at fat reduction than steady state exercise and also increases the levels of circulating testosterone.

 

6.  Lift Heavy Weight

There are numerous resistance-training programs you can follow; hypertrophy plans, strength plans, strength and endurance plans, body weight workouts, rage workouts and more.  All of these plans will cause a specific adaptation to the imposed demand.  Resistance training or weight training is a potent stimulator for testosterone production.  Be sure to include some form of weight training 2-3 times per week to help boost your body’s natural production of testosterone.  Interestingly, rest periods of 90 seconds between sets has the greatest effect on post workout testosterone and growth hormone levels.

 

7. Avoid Chronic Stresses

It is vitally important to rest your body as well.  Adequate rest promotes recuperation and recovery.  Excessive training, stress, and/or inadequate amounts of sleep contribute to elevate cortisol levels, which is antagonistic to and reduces testosterone levels.  Appropriate rest and recovery also promotes healthy growth hormone levels.  Also, avoid too much of a good thing.  Excessive or chronic training can lead to overtraining syndrome, which is counterproductive to a balanced hormonal environment.

 

For many of us, despite following all of these recommendations, you may still suffer from testosterone deficiency from one of the many causes or conditions:

  • Viral Infection
  • Radiation/Chemotherapy
  • Genetic conditions
  • Leydig cell dysfunction
  • Various Medications
  • Excessive Alcohol
  • Aging
  • Hemochromatosis
  • Chronic Systemic Disease
  • Diabetes
  • Chronic Obstructive Pulmonary Disease
  • Metabolic Syndrome
  • Tumors
  • Environmental Toxins

 

Testosterone deficiency is associated with the onset and/or worsening of these medical conditions:

  • Cardiovascular disease
  • Dementia
  • Bone demineralization
  • Joint & Tendon degeneration
  • Diabetes & Metabolic Syndrome
  • Depression
  • Vascular endothelial dysfunction
  • Increased Carotid intima media thickness and vaso-reactivity
  • Obesity

 

If you are following a healthy nutrition plan, exercising regularly, drinking plenty of water, getting adequate rest and recovery but are still noticing these symptoms, then you may be suffering from testosterone deficiency despite doing all the right things.  :

  • Poor memory, concentration, or focus
  • Moody or depressed
  • Lack of motivation or drive
  • Lack of stamina
  • Poor recovery or increased recovery time from exercise
  • Loss of muscle mass or difficulty maintaining muscle mass
  • Increasing belly fat or difficulty losing belly fat despite exercising
  • Fatigue especially in the afternoon or after eating
  • Poor libido
  • Decreasing erectile quality
  • Loss of hair or reduced shaving
  • Hot flushes or sweats

The first step is to get the correct lab work and have a physician experienced and knowledgeable in the diagnosis, treatment, and management of male hormone deficiencies and all of its various nuisances and intricacies.

Testosterone deficiency presents with a recognizable symptom complex that is highly amenable to hormone therapy if a reliable and predictable treatment plan is consistently administered.

Testosterone is supremely important for the health and vitality of men and women.  Optimizing its production by following these seven vital steps will help to ensure your personal health and well-being.

What To Know Before You Treat Low Testosterone

Feeling like a shadow of your former self?  Gaining fat weight around the midsection?  Not so interested in the things that were once your passion? No longer feel focused and mentally alert?  Could care less about sex?  Hormone imbalance could be the issue and Testosterone Deficiency or “Low T” may be robbing you of your once dominant youth and vitality!

The number of men diagnosed with “Low T” has grown dramatically in recent years due to an increasing awareness of the importance of hormones in a man’s health and well being.  Research shows that about 1 out of 4 men over the age of 30 may have Low-T.  Circulating testosterone levels decline progressively with age, starting in the second and third decade of life, owing to defects at all levels of the hypothalamic-pituitary-testicular axis.  Your doctor may consider prescribing testosterone if you report classic signs and symptoms of testosterone deficiency and if appropriate lab tests confirm that your hormone levels are low. But testing for testosterone deficiency requires a comprehensive understanding of the intricacies of hormone balance before one makes a commitment to what may be lifelong therapy.

Inaccurate, misinterpreted lab results or lack of diagnostic acumen can lead to either a false diagnosis or misdiagnosis of a case of testosterone deficiency. And because Low–T treatment is not without risk, it is important to have your testosterone levels and associated hormones accurately assessed before considering treatment.

Often men are referred to the our practice with only a single lab value ordered by their physician: Total testosterone.  This single lab value, while part of the diagnostic puzzle is insufficient alone to make an accurate diagnosis or to determine a potentially curable underlying cause to testosterone deficiency.

Below are 7 steps to consider before starting Low-T therapy.

Step One: Find the Right Doc

Finding the right doctor can be tricky, however there are many physicians that specialize in the diagnosis, treatment, and management of men with hormone deficiencies.  If your doctor does not have you do the basics such as completing an ADAMS Questionnaire, a Lifestyle Questionnaire and a Nutrition Questionnaire then it is time to get up and walk out.  Obtaining a clear diagnosis that determines testosterone deficiency and discovering if there is a treatable underlying cause to the Low T is essential to your long term health and well being.

Work with a physician who understands the complexities of diagnosing, treating and managing Low T.  You’ll need a game plan much like the head coach of a professional sports team.  If a care provider wants to give you testosterone replacement therapy without supervision and follow-up, find a new head coach.

A dutiful and careful evaluation will involve laboratory testing for testosterone, associated hormones, and screening labs on more than one occasion. I encourage men to not be shy about seeking the opinion and care of a physician who specializes in men’s hormone health.

Even when lab tests don’t show clear testosterone deficiency, the final decision to offer treatment is a judgment call based on many factors. If a man has a symptom complex consistent with testosterone deficiency, a low-normal testosterone level and a decrease in quality of life then it is appropriate to consider discussing the risks and benefits of the various treatment options to see if this may help alleviate some or all of his symptoms.

Step Two:  Identify a Known Symptom Complex

The first step in diagnosing testosterone deficiency is establishing that a man has the classic symptoms associated with Low-T. This requires utilizing the ADAMS questionnaire as well as an intensive Nutrition and Lifestyle questionnaire to discover underlying contributing factors. Rarely does a deficiency or excess in a single hormone or pro-hormone contribute to all the symptoms. Guidelines released in 2010 by the Endocrine Society cited a list of symptoms that raise the possibility of Low T. These are low libido, loss of body hair, hot flashes, poor concentration and memory, insomnia, decreased energy, motivation, initiative, self-confidence, decreased muscle mass, increased abdominal fat and diminished physical or work performance.  Rarely does a man have all of these symptoms.  At Renue Health the initial evaluation/consultation to assess a man’s current hormone status takes about 2 hours.  It’s just not possible to do this in a 15-minute office visit.

Step Three:  Get the Right Labs

Getting the correct lab requires seeing a physician who specializes in the treatment of male hormone deficiencies.  So often I see men who have been to a previous care provider who has made a diagnosis solely from a single lab value, total testosterone.  A single value of total testosterone by itself has very little diagnostic value and the main issue with total testosterone is that only a small fraction, about 1%-2% that is in the blood stream is biologically active.  About 50% of the remaining hormone is attached to albumin and is potentially available to be put to use.  However, anywhere from 40%-70% of the total testosterone is bound to a protein called sex hormone binding globulin (SHBG).  The affinity of testosterone and SHBG is strong.  This results at times to a large portion of total testosterone actually being biologically unavailable, even with what appears to be a “normal” total testosterone level.  An intensive laboratory panel must be ordered searching for treatable underlying causes to the deficiency.

Step Four:  Fix Your Digestive System

An aging gut or toxic gut equals a leaky gut.  And a leaky gut leads to inflammation, a poor immune system and hormone imbalance.  Stress and toxins that pollute your digestive system make it impossible for the body to be in balance.   In order to restore hormone balance you must repair the gastric and intestinal lining, replace the digestive enzymes, re-inoculate with beneficial bacteria, and restore fiber to your nutrition regularly.  A doc familiar with the intricacies in the optimizing of your testosterone must be familiar with the steps necessary to heal your GI tract.

Step Five: Detox Your Liver

November 11th is Veterans Day – a time to honor those who served in our nation’s military. On this and every day, let’s bring awareness to a major issue many returning Veterans face: Traumatic Brain Injury, or TBI.

Today on Living Dayton we discussed the staggering number of Veterans that suffer from Traumatic Brain Injuries (TBI) and PTSD. These two conditions often accompany each other, reflected in the lives of our soldiers returning from the Iraq and Afghanistan wars. The Dangers Of Gas Station Viagra

Male sexual enhancement is a widely used term, covering everything from libido to Erectile Dysfunction, even increases in size. The multi Billion dollar market of little blue pills like Viagra and supplements is an overwhelming indicator that men desire the benefits of these products. While they can help increase desire and obtain longer lasting, higher quality erections, they are not without their risks.

The most common pharmaceuticals for erectile dysfunction are Viagra, Cialis and Levitra. These are a class of drugs known as PDE-5 (Phosphodiesterase type 5) inhibitors. For the sake of this article, we will be discussing Viagra and the knock off, nonprescription, over the counter herbal supplements often found at gas stations and adult novelty stores.

 

How do PDE-5 inhibitors like Viagra work?

A common misconception is ED medications cause a man to be sexually aroused. While they do not cause arousal, they do work when sexual arousal is achieved. This is because sexual stimulation sends a signal to the brain to trigger a release of nitric oxide (NO) in the tissues of the penis.

The nitric oxide stimulates an enzyme that produces something known as a messenger cyclic guanosine monophosphate (cGMP), relaxing the smooth muscle cells. Sildenafil selectively blocks PDE-5, allowing the erection enhancing cGMP to remain longer and not be broken down. This increased cGMP allows extra blood flow to the penis. The result: Firmer, longer lasting erections.

To have great sex you need abundant energy, high aerobic capacity, strength, stamina, mobility, health and above all: blood flow. One result of increased nitric oxide is that the arteries in your penis dilate and the blood can flow into your penis more easily. Another result is that the erectile tissue itself fills with blood. Both processes result in an erection. Viagra works by maintaining the level of cGMP in the smooth muscle cells, allowing more blood into the penis.

 

The Herbal Viagra

The FDA warns: “Sexual enhancement products that claim to work as well as prescription products are likely to contain a contaminant. Use of such products exposes consumers to unpredictable risk and the potential for injury or even death.”

Many over the counter supplements contain non FDA approved sulfoaildenafil, a compound structurally similar to sildenafil. Some even contain sildenafil, which is FDA approved and the active ingredient in the prescription drug Viagra. The trouble is, the source of sildenafil in these OTC supplements is often from an overseas laboratory and not from an FDA approved facility. Both drugs are used to treat Erectile Dysfunction (ED) and pose the risk of interaction with nitrates found in some prescription drugs such as nitroglycerin. This interaction may lower blood pressure to dangerous levels. Men with diabetes, high blood pressure, high cholesterol, or heart disease often take nitrates.

Other dangers include undisclosed amounts of the active ingredient (sildenafil or sulfoaildenafil), which in high amounts can lead to a permanent, painful erection, damaging the penile tissues. Some herbal sexual supplements also contain ingredients such as saw palmetto, ginseng, yohimbine and horny goat weed. The amounts of these ingredients are often undisclosed and can pose risk of interaction with other medications as well as raise blood pressure and heart rate to unsafe levels.

 

Beware of Male Enhancement products that:

  • Promise quick results (within 30 to 40 minutes)
  • Are advertised as alternatives to FDA-approved prescription drugs
  • Are sold in single servings
  • Advertise via spam or unsolicited emails
  • Have labels written primarily in a foreign language
  • Have directions and warnings that mimic FDA-approved products

The End Game of Hormone Optimization

The goal of the hormone optimization is to improve one’s quality of life and to create an internal healthy environment that provides the lowest possible risk for the various diseases associated with aging and declining hormones. This is accomplished through early detection, identification of risk factors, and proactive strategies designed to prevent disease as much as possible, rather than treating disease once it is established.

Treatment for hormone optimization is dutifully centered on 4 basic principles:

  • Low glycemic nutrition
  • Appropriate meaningful and adaptive exercise
  • Specific nutritional supplementation
  • Correction of metabolic and hormonal deficiencies as medically indicated.

The correction of hormonal deficiencies, specifically testosterone, is a topic that draws the most attention and controversy. Is taking hormones safe? Do hormones cause cancer? Do hormones really help or do they just make us feel better? Many physicians who are not familiar with the recent medical literature on the topic feel that correcting hormone deficiencies is not particularly important, and that it may be potentially harmful.

The fact is that hormones decline with age. Most men run into their archenemy in their late thirties, forties and fifties. A slow and gradual decline that is hardly perceptible for most men. Contrarily, for women their nemesis confronts them rather abruptly over a three to five year period. Recently, I had a 32 year old symptomatic patient who’s free testosterone was 37, which is incredibly low and a 70 year old who’s Free T was 210, fantastic! The point here is that hormones decline according to their own schedule. You can submit to this unnecessary, treatable disease or you can Take Action when it is your time!

This inevitably decline in hormones is associated with many disease processes including heart disease, diabetes, Alzheimer’s dementia, abdominal obesity, loss of lean muscle, and osteoporosis. Correction of these deficiencies does not stop aging. However, it can delay the onset of many of the diseases associated with aging and slow their progression, as well as improve an individuals overall quality of life. And for me personally, what is life, if there is no quality.

Here are 20 supporting reasons in the medical literature to optimize your hormones:

“Aging in men is characterized by a progressive decline in levels of anabolic hormones, such as testosterone, IGF-1 (growth hormone), and DHEA. Age associated decline in anabolic hormone levels is a strong independent predictor of mortality in older men. Having multiple hormonal deficiencies rather than a deficiency in a single anabolic hormone is a robust biomarker of health status in older persons.” (Archives of Internal Medicine, 2007)

“…Testosterone concentrations are inversely related to mortality due to cardiovascular (heart) disease and all causes.” (Circulation, the Journal of the American Heart Association, 2007)

“In older men, lower total testosterone levels predict increased incidence of stroke or TIA (mini stroke) after adjusting for conventional risk factors for cardiovascular disease. Men with low-normal testosterone levels had increased risk.” (Journal of Clinical Endocrinology and Metabolism, 2009)

“Testosterone replacement therapy reduces insulin resistance and improves glycemic (glucose) control in hypogonadal (low testosterone) men with type 2 diabetes. Improvements in glycemic control, insulin resistance, cholesterol and visceral adiposity (belly fat) together represent an overall reduction in cardiovascular risk.” (European Journal of Endocrinology 2006)

“In men with low testosterone levels, testosterone treatment was associated with decreased mortality compared with no testosterone treatment.” (Journal of Clinical Endocrinology and Metabolism, 2012)

“Low free testosterone is an independent risk factor for Alzheimer’s disease.” (Experimental Gerontology, 2004)

“Administration of testosterone to hypogonadal (low testosterone) men reverses part of the unfavorable risk profile for the development of diabetes and atherosclerosis” (hardening of the arteries). (Journal of Obesity, 2011)/p>

“In patients with coronary (heart) disease, testosterone deficiency is common and impacts significantly negatively on survival.” (Heart, 2010)

DHEA-S concentration is independently and inversely related to death from any cause and death from cardiovascular disease in men over age 50. (New England Journal of Medicine)

Low serum levels of DHEA-S predict death from all causes (Journal of Clinical Endocrinology and Metabolism 2010) Higher DHEA-S levels are independently and favorably associated with executive function, concentration, and working memory. (Journal of Clinical Endocrinology and Metabolism 2009)

“Blood levels of androgens (like testosterone) and other sex hormones do not seem to be related to the risk for prostate cancer. The finding comes from a huge pooled analysis of data from 18 studies, published in the Journal of the National Cancer Institute (that) confirms the lack of evidence to support an androgen–prostate cancer hypothesis.”

Urologist, leading expert, and Harvard Faculty, Dr. Abraham Morgantaler, states that “there is not now – nor has there ever been – a scientific basis for the belief that testosterone causes prostate cancer to grow” (European Journal of Urology, 2006)

Low blood levels of testosterone do not protect against prostate cancer.

Low testosterone and age are the greatest risk factors for prostate cancer.

High blood levels of testosterone do not increase the risk of prostate cancer.

Treatment with testosterone does not increase the risk of prostate cancer, even among men who are at high risk.

“Mortality due to malignancies (cancers) was not elevated in adults receiving hGH treatment.” (Journal of Clinical Endocrinology and Metabolism 2011)

With respect to hGH (human growth hormone), which is only recommended for patients who have been shown to be deficient based on strict FDA guidelines:

“…Relatively high circulating IGF-I (growth hormone) bioactivity in elderly men is associated with extended survival and with reduced cardiovascular risk.” (Journal of Clinical Endocrinology and Metabolism 2008)

“Like HDL (good cholesterol), high (normal) levels of IGF-1 (growth hormone) confer protection against coronary artery disease.” (Atherosclerosis. 2011)

The aforementioned excerpts are only a small selection of the medical literature that supports the safety and efficacy of replacing and optimizing hormone deficiencies. My goal is to provide patients with the most up to date medical evidence to help them make informed decisions that guide them to their optimum health and wellbeing.

The monumental issue in addressing the optimization of hormones, especially low testosterone, is that it is a legitimate treatment for a medical condition that affects all persons at various times in their life. Quite different than the illicit and hazardous use by amateur and professional athletes that is portrayed almost daily by the uneducated and misinformed media dramatizing stories for viewer ratings. The notion that testosterone is evil, the cause of male aggression, or dangerous needs to be surrendered. The optimization of testosterone and all hormones under the supervision of a physician is treatment for a medical disease. Restoring hormones to optimum levels corrects deficiencies, is proactive and preventative for many disease processes, and helps the individual regain their full, normal potential and the highest quality of life. And a high quality of life is what we all strive to achieve.

Thyroid hormone is a metabolic hormone secreted by the thyroid gland that regulates temperature, metabolism and cerebral function. Is Emotional Eating Sabotaging Your Weight Loss Goals?

Food has such a powerful place in all of our lives. It’s how we bond with our families as we grow. Food has very powerful cultural and ethnic roots in all our societies. It’s there when we celebrate and it’s there when we mourn. When you stop to think about it, eating and food is the one thing that we do every single day where we use all of our senses.

We see the bright red color of a delicious apple, we smell the aroma of cinnamon and crust from a freshly baked hot apple pie, we taste that sweet sugary goodness of the pie and feel the texture of it in our mouths and we are aware of our surroundings filled with family, friends, and hear the sounds of laughter. When you are using all of your senses, you are truly alive. Do you wonder now why food has such strong, powerful, emotional connection to all of our lives?

However, emotional eating can sabotage your weight-loss efforts permanently if you are not careful. Emotional eating often leads to eating too many high-calorie, sweet, and sugary foods. The good news is that if you are prone to emotional eating, you can take steps to gain control of your eating habits and get control of your life and your weight.

Emotional stress eating

Emotional eating is a way to deal with negative emotions.

Emotions such as anger, anxiety, boredom, fear, sadness, jealousy, stress, and loneliness can all trigger a feeding frenzy. Just the hassles of daily life can produce negative emotions that lead to emotional eating, which can destroy all of your efforts. Getting control over these triggers is possible, and it starts with identifying them.

These are some very common reasons for emotional eating:

  • Loss of a job
  • Death of a loved one
  • Financial pressure
  • Illness
  • The “D” word
  • Work stress
  • Boredom
  • Fatigue
  • Getting Married
  • Strained relations with family and friends

Not everyone displays emotional eating the same. Some people actually eat less when challenged by strong feelings. Others in emotional distress may turn to binge eating, rapidly eat whatever is available without ever even taking the time to enjoying it. Your emotions may become so hard-wired to your eating habits that you automatically reach for junk food whenever you feel stressed without stopping to think about what you are even doing.

Food is a wonderful distraction from the stresses of everyday life. And it is easy to find comfort and pleasure in all that sugary goodness when you perceive that things in your life are not going well. If you’re worried about an upcoming event such as a wedding or stewing over a relationship conflict you may focus on eating comfort food instead of dealing with the painful situation, which requires healthy coping skills, emotional strength, and delayed gratification.

Whatever underlying emotion drives you to overeat, the end result is always the same. The emotions return, and now you bear the additional burden of guilt about failing to keep your weight loss commitment. This can lead to an unhealthy cycle. Your emotions trigger you to overeat, you beat yourself up for overeating and falling off your dietary plan, you feel badly, and what do you do, you overeat again.

So what can you do to stop the emotional eating?

Making good food choices

I am hear to tell you there is hope, and it all starts with awareness of your behavior! Negative emotions can trigger emotional eating but the good news is you can take steps to control the feeding frenzy and keep your weight loss efforts moving forward. Here are some useful suggestions to help control emotional eating:

Reduce your stress. If stress contributes to your emotional eating try exercise like yoga or breathing relaxation. Also, dealing with the stressor head on and solving the issue rather than letting it smolder in the background is always a sound strategy.

Are you really hungry? If you ate just a few hours ago and do not have any physical signs of hunger than you are probably not really hungry. Drink a full glass of cold water first and give the craving a little time to pass.

Keep a food diary. Write down what you eat, how much you eat, when you eat, how you are feeling when you eat. Over time you will see patterns emerge that reveal the connection between your mood and food that you choose to eat.

Find support. You are more likely to suffer from emotional eating if you lack a good support network. Looks towards your family and friends or consider joining a support group. Associate with other health focused people.

Fight boredom. Instead of snacking when you are not hungry. Discover an activity to distract yourself. Take a walk, listen to upbeat music, watch a movie, read a book, surf the Internet or call a friend.

Remove the food. Eliminate supplies of comfort foods in your home. And if you feel emotional delay your trip to the grocery store until you have calmed down.

Reward yourself. It is important to reward yourself for the goals you accomplish along the way. It is neither reasonable nor healthy to restrict your calories for too long a time period. Having a favorite food or meal once every week while trying to loose weight is good for your body and soul.

Eat healthy snacks. If you get hungry between meals choose a snack such as apples, berries, or celery. Or try low-fat cottage cheese with fruit.

Get enough sleep. Getting adequate sleep is important to decrease cravings for carbohydrates and sugary junk food. Some of my worst food frenzies came when I was stressed and exhausted from being over worked. Learn to relax and rest your body every night.

Consider counseling. If you have tried these options and you still can’t get control of your emotional eating, I would consider seeking therapy with your health care provider. Emotional eating therapy can help you understand the motivating factors behind your emotional eating and help you discover new coping skills. If you have an episode of emotional eating, learn from the experience and the very next meal eat healthy.

Brett Favre Opens Up About His Traumatic Brain Injuries

Last week, Brett Favre spoke with Megyn Kelly about the state of his mental health today after receiving many concussions over his two decades of playing football.

In the interview, Brett describes his cognitive decline, something he thought would never happen. His experience ranges from increased forgetfulness to impaired motor function. These are common symptoms associated with a TBI, a condition often called a concussion. Today, brain injuries are something we now understand how to diagnose and treat. The better news is we are able to cure those that suffer a brain injury, making getting your life back possible!

More on Traumatic Brain Injury/Concussion

Results of recent and ongoing studies have made it clear that brain injuries like Traumatic Brain Injury (TBI) pose substantial risk to pituitary function, multiple hormones and overall health and wellness. Hypopituitarism may benefit with appropriate hormonal replacement receiving replacement therapy such as antidiuretic hormone, glucocorticoid, and thyroid hormones when needed.

Testosterone, estrogen, progesterone, pregnenolone, DHEA, and recombinant human growth hormone replacement therapy should also be introduced as indicated.  Signs and symptoms of post-TBI hypopituitarism are often masked by what has been assumed to be post-traumatic syndrome.  It is possible to improve the quality of life and enhance the post traumatic brain injury period with hormone replacement therapy and supplementation that addresses neuro-inflammation.

Clinical evidence has demonstrated that TBI may frequently cause hypothalamic-pituitary dysfunction, probably contributing to a delayed or hampered recovery from TBI.  Changes in pituitary hormone secretion may be observed during the acute phase post-TBI.  Diminished pituitary hormone secretion, caused by damage to the pituitary and/or hypothalamus, may occur at any time after TBI.

The most common alterations appear to be gonadotropin and somatotropin deficiency, followed by corticotropin and thyrotropin deficiency.

In order to improve outcome and quality of life in individuals who have suffered a TBI an adequate replacement therapy program is of paramount importance.

The Effects of TBI

Most people are unaware of the scope of TBI or its overwhelming nature. TBI is a common injury and may be missed initially when the medical team is focused on saving the individual’s life. Before medical knowledge and technology advanced to control breathing with respirators and decrease intracranial pressure, which is the pressure in the fluid surrounding the brain, the death rate from traumatic brain injuries was very high. Although the medical technology has advanced significantly, the often overlooked effects of TBI are significant.

TBI is classified into two categories: mild and severe

A brain injury can be classified as mild if loss of consciousness and/or confusion and disorientation is shorter than 30 minutes. While MRI and CAT scans are often normal, the individual has cognitive, physical, and social problems such as:

  • fatigue
  • headache
  • difficulty thinking
  • memory problems
  • brain fog
  • attention deficits
  • mood swings
  • frustration
  • muscle weakness
  • exercise intolerance
  • weight gain
  • isolation
  • depression
  • anxiety
  • panic attacks
  • social phobias

These injuries are commonly overlooked.  Even though this type of TBI is called “mild”, the effect on the family and the injured person can be devastating.

Severe brain injury is associated with loss of consciousness for more than 30 minutes and memory loss after the injury or penetrating skull injury longer than 24 hours. The deficits range from impairment of higher-level cognitive functions to comatose states. Survivors may have:

  • limited function of arms or legs
  • abnormal speech or language
  • loss of thinking ability
  • emotional problems

The range of injuries and degree of recovery is very variable and varies on an individual basis.

The effects of TBI can be profound. Individuals with severe injuries can be left in long-term unresponsive states. For many people with severe TBI, long-term rehabilitation is often necessary to maximize function and independence.  Even with mild TBI, the consequences to a person’s life can be dramatic. Change in brain function can have a dramatic impact on family, job, social and community interaction.

So how do we treat a Traumatic Brain Injury?

Treating Joint Injuries With PRP Podcast

Recently, I sat down with Jay Campbell to discuss the many ways we can heal the body through the use of PRP (Platelet Rich Plasma), Stem Cells, and Peptides. All of these are naturally present in the body, and with carefully targeted treatment we can give someone with joint injuries an alternative to surgery with minimal discomfort and almost no downtime.

Platelet Rich Plasma (PRP) For Joint Injections

We use Platelet-rich plasma (PRP) in our practice to treat tendon irritations, tendinitis, and bursitis around the shoulder, knee, elbow, and ankle joints.

PRP Blood DrawWe start by drawing a small amount of a patient’s blood, then spin it in an orthopedic centrifuge to isolate the PRP. The doctor then injects the PRP directly into the injured area, with the use of either ultrasound or fluoroscopy, where the growth factors can take action to help heal the damaged tissue.

The PRP has the benefit of stimulating the release of stem cells from vessel walls further augmenting repair. Stem cells are also being directly added to our cartilage regeneration efforts.

Tendinitis is an inflammation, degeneration or injury to the tendon, the structure in your body that connects muscle to bone. Tendinitis can lead to degeneration of the collagen fibers that make up the tendon. The chronic pain that develops with tendinitis has been successfully treated with soft tissue massage, stretching, icing, and careful exercises.

In the past, when tissues and joints were injured, the common treatment in similar circumstances was a cortisone injection. Cortisone is a steroid that inhibits protein metabolism so inflamed cells that are actively turning over are shut down. This effect reduces swelling and therefore pain. Unfortunately, it also reduces activity in normal cells and tissues, and leads to weakening of the tissues and occasional tendon ruptures. Joints that are repeatedly injected with cortisone show damage to the articular cartilage surfaces, which protect the ends of bones. Rather than work to actively heal the damaged tissue, a cortisone injection unfortunately only serves to mask the injury, allowing the athlete to continue to play, often causing more damage.

Steps for the joint injection procedure

1. Identifying the area of concern.

Using an ultrasound machine, we examine the area of concern to locate the damaged tissue.

2. Isolating and activating the magic from your blood.

First, the doctor draws about 2 teaspoons of blood. Using a centrifuge in our office, we isolate platelets from that blood (platelet rich plasma, or PRP), all within about 15 minutes. The platelets are then “activated” to release at least eight growth factors will be used to heal injured tissue. These growth factors work like magic to cause increased collagen and new blood flow.

3. Injecting the PRP into the joint.

Using numbing cream and a very small needle the doctor injects the Platlet Rich Plasma containing a high concentration of growth factors into the joint. These growth factors then activate multipotent stem cells already in the tissue, tricking them into “thinking” there’s been an injury. By creating an inflammatory response, the body sends more of it’s own healing elements to the area. The result is the generation of new, younger tissue to replace that which was torn or damaged.

 

What joint injuries can we treat with PRP?

Shoulders

Traditional options for patients suffering from many shoulder injuries include arthroscopic shoulder surgery to repair tears, or shoulder joint replacement. With both surgeries, months of rehab are required, and the patient must be aware of and prepared to take on the risks. As an alternative, PRP Injections can be performed with promising results for recovery without the need to undergo surgery.

Knees

Surgical options for those dealing with painful knee issues range from arthroscopic knee surgery to total knee joint replacement. Knee replacement is extremely traumatic and carries new-found risks of toxic wear particles entering the blood stream. Surgeries also often require months of painful rehab to regain strength and mobility. Most surgeries also accelerate the cascade of degeneration that leads to osteoarthritis. Depending on the severity of a knee injury, PRP may be a great alternative to surgery.

Ankles

Current surgical treatment options for common injuries and arthritis, include ankle arthroscopy to “clean up” damaged cartilage, fusion of ankle bones where cartilage has been lost, and tendon grafts screwed in place to strengthen loose connections between bones. All are painful and may not be effective in the long run. Depending on the severity of ankle injury, PRP may be a great alternative to surgery.

If you are experiencing elbow pain, PRP is likely an excellent treatment option. Do It Yourself Super Sports Drinks

One of the most misleading industries is the Sports Drink Industry. The purpose of a sports drink is to replenish the electrolytes that can be lost during either intense or endurance training. However, as much as we may like to believe, the majority of us never engage in activity that is strenuous enough to deplete electrolytes to the point that would require the replenishing of base electrolytes. Then, we negate any of the positive effects of exercise by consuming more calories in the sports drink than calories burned during exercise.

In the majority of sports drinks we found the major components are high fructose corn syrup, water, salt, potassium citrate, sucralose, sodium citrate, malic acid, potassium phosphate, phenylalanine, and minute amounts of B vitamins. Other than the B vitamins, your body does not require the other ingredients. When these other ingredients are consumed in excess they lead to weight gain and several lifestyle diseases. And in my medical opinion, these drinks are nothing more than “Junk Liquid”.

In general, consuming sports drinks after exercise lasting less than 60 minutes will only be counterproductive because you will most likely drink more calories than what you utilized during the exercise session. Your body is highly efficient at conserving electrolytes. Your best option and least expensive is to drink plenty of water before, during, and after any exercise session.
